Embryo transfer day is filled with hope. It’s the moment everyone has been working toward—intended parents, surrogates, medical teams, and agencies alike. But what happens after the transfer? Whether the result is a positive pregnancy test or not, each path forward is met with care, clarity, and compassion.

💖 When the Transfer Works

A positive pregnancy test—typically done 10–14 days after transfer—is a moment of celebration. It means the embryo has implanted and the surrogate is officially pregnant.

Here’s what happens next:

  • Beta hCG levels are monitored through bloodwork to confirm the pregnancy is progressing.

  • Follow-up ultrasounds are scheduled to check for a heartbeat and ensure everything is developing as expected.

  • The surrogate continues medications as directed and transitions into early prenatal care.

  • Around 10–12 weeks, care is typically transferred from the fertility clinic to the surrogate’s OB provider.

This phase is filled with joy, gratitude, and cautious optimism. It’s also a time when the relationship between surrogate and intended parents deepens, as they begin to share the journey of pregnancy together.

💛 When the Transfer Doesn’t Work

Sometimes, despite everyone’s best efforts, the transfer doesn’t result in a pregnancy. This can be heartbreaking—but it’s not the end of the journey.

Here’s what happens next:

  • The surrogate will stop medications as directed by the clinic.

  • The medical team may review the cycle to understand what factors may have contributed.

  • A follow-up call or appointment is often scheduled to discuss next steps.

  • If all parties are emotionally and medically ready, a new transfer cycle may be planned.

It’s normal to feel disappointment, grief, or even confusion. That’s why emotional support is so important during this time. Surrogates and intended parents are encouraged to lean on their agency, counselors, and each other. Every journey is unique, and healing looks different for everyone.
